Crash reports

Due to their confidential nature, Texas crash reports are not available for online viewing by the general public in accordance with Texas law.

TxDOT offers these formats for submitting crash data:

  • Crash reporting and analysis for safer highways (CRASH): The CRASH application was created to allow law enforcement officers to submit the Texas Peace Officer’s Crash Report (form CR‐3) electronically to the state of Texas. Please visit our CRASH system page for more details.
  • Submission services: Submission services was developed for agencies with an existing application and requires a web services client and adherence to the over 800 business rules and edits. If interested in this option, please provide your vendor name, contact for your vendor and an agency contact to discuss implementing this option.

Texas crash report forms

This page contains the crash report forms and instructions used to report all crashes. Due to TxDOT’s 10-year retention policy for crash reports, and changes to the forms over time, there are five groups of crash reports and instructions to be used by law enforcement agencies when reporting crashes. Per TxDOT retention policy, crashes that occurred on Dec. 31, 2013 or before will no longer be accepted into TxDOT’s custodial records.

  • 2023 Texas Peace Officer’s crash report forms – To be used for crashes that occurred on or after April 1, 2023.
  • 2018 Texas Peace Officer’s crash report forms – To be used for crashes that occurred between Jan. 1, 2018 and March 31, 2023.
  • 2017 Texas Peace Officer’s crash report forms – To be used for crashes that occurred between Jan. 1, 2017 and December 31, 2017.
  • 2015 Texas Peace Officer’s crash report forms – To be used for crashes that occurred between Jan. 1, 2015 and Dec. 31, 2016.
  • 2010 Texas Peace Officer’s crash report forms – To be used for crashes that occurred between Jan. 1, 2014 and Dec. 31, 2014.

TxDOT previously offered the CR-3 Alternate Texas Peace Officer’s crash report form that was formatted to include the crash report codes within the report. This form should only be used to supplement crash reports originally filed on a CR-3A.

Note: The CR-3 and CR-3 Alternate forms are for use by Texas law enforcement agencies only.

2023 Texas Peace Officer’s crash report forms

The 2023 forms and publications are to be used to report all reportable crashes occurring on or after April 1, 2023. Note: This includes supplemental reports for crashes that occurred during this time period.

2018 Texas Peace Officer’s crash report forms

The 2018 forms and publications are to be used to report all reportable crashes occurring between Jan. 1, 2018 and March 31, 2023. Note: This includes supplemental reports for crashes that occurred during this time period.

Note: The CR‐3 Alternate form was not updated for 2018. All Texas law enforcement agencies must use the CR‐3 form for 2018 and later crashes.

2017 Texas Peace Officer’s crash report forms

The 2017 forms and publications are to be used to report all reportable crashes occurring between Jan. 1, 2017 and December 31, 2017. Note: This includes supplemental reports for crashes that occurred during this time period. The 2017 forms include updated Driver License Restriction code tables, added by Texas Department of Public Safety in December 2016.

Note: If the CR-3 Alternate form is used, please use to the 2015 Texas Peace Officer’s Crash Report Forms section below as no changes were made to these forms in 2017.

2015 Texas Peace Officer’s crash report forms

The 2015 forms and publications are to be used for all reportable crashes that occurred between Jan. 1, 2015 and Dec. 31, 2016. Note: This includes supplemental reports for crashes that occurred during this time period.

2010 Texas Peace Officer’s crash report forms

The 2010 forms and publications are to be used for all reportable crashes occurring between Jan. 1, 2014, and Dec. 31, 2014. Note: This includes supplemental reports for crashes that occurred during this time period. Per TxDOT retention policy, crashes that occurred on Dec. 31, 2013 or before will no longer be accepted into TxDOT’s custodial records.

Other crash records forms

Important Note: Effective Sept. 1, 2017, per the 85th Texas Legislature Senate Bill 312, the Driver’s Crash Report (Form CR‐2) is no longer retained by TxDOT, and TxDOT no longer hosts or provides copies of the CR-2 form.
