Crash Reports and Records

Confidentiality

Due to their confidential nature, crash reports are not available for online viewing by the general public.

Individual reports may be obtained by clicking on the appropriate link below and furnishing at least two out of the three pieces of information required by statute.

Crash Reports

We collect crash reports submitted by law enforcement officers and reports submitted by drivers. Both are available to the public for a processing fee.

  • Texas Peace Officer's Crash Report
    This report is completed by a law enforcement officer investigating a traffic crash when apparent damage is $1,000 or more, or when the crash resulted in injury or death.
     
  • Driver's Crash Report (Blue Form)
    This report is completed by a driver involved in a traffic crash when the crash is not investigated by a law enforcement officer and apparent damage is $1,000 or more or when the crash resulted in injury or death.

Required Information

Under state law, before crash reports can be released the requester must provide at least two of the following pieces of information:

  • Date of the crash
  • Location of the crash
  • Name of any person involved in the crash

Costs and Ordering

  • $6.00 for a regular copy
  • $8.00 for a certified copy

A certified copy is requested when a person needs an official document, such as for a legal proceeding. To obtain a copy of either report, please complete the appropriate request form and mail it with the required payment to the address shown on the form.

Crash Records

TxDOT is also responsible for the collection and analysis of traffic crash data including:

  • collecting and maintaining crash reports submitted by law enforcement and drivers,
  • classifying traffic crashes in accordance with national standards,
  • entering the information from each report into the Crash Records Information System, and
  • providing copies of crash reports as requested by the public.
